University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant who will deliver care for patients on the University of Minnesota Heart Care- Heart Failure team at the University of Minnesota Medical Center and the Clinics and Surgery Center (CSC) in Minneapolis. The position will provide patients with expert patient care and coordination of services. They will assist the practice to improve patient satisfaction and enhance the quality of care provided to patients through increased access and coordinated services. In This Role, You Will:
