Job Summary:

The Nurse Navigator serves as the primary point of contact for physicians and other healthcare providers, patients and their caregiver/family, and other resource-based entities, providing and accessing clinical and supportive care services offered within the multidisciplinary teams at Baptist Health Hardin and other entities as needed. The Nurse Navigator serves as an integral part of the care team within the Baptist Health Hardin Cancer Care Services department, coordinating effective and comprehensive services through the continuum of care for patients from the time of all breast-related diagnoses (cancer-related or suspicious findings), with treatment and support service options and through the recovery phase of care. The Nurse Navigator serves as an educator, advocate, and communicator for the patient and patient caregiver/family and proactively develops processes that streamline and improve the clinical experience for both the patient and their healthcare provider(s). The Nurse Navigator will demonstrate through organizational skills the ability to provide ease and patient safety in the operational flow of the accessed Baptist Health Hardin departments. Additionally, the Nurse Navigator will also work closely as a liaison throughout Baptist Health Hardin and in the hospital's service region with Marketing/Community Outreach/Health & Wellness educating all parties on the services provided at Baptist Health Hardin for prevention, screening/detection of breasts-related diagnoses, treatment programs and palliative services for the affected patient population. The Nurse Navigator demonstrates the broad-based knowledge, skill, and coordination to provide nursing care and guidance to the cancer patient from screening to survivorship in a manner that is supportive of Baptist Health Hardin's mission and vision statements. This position requires a collegial and collaborative working style to fulfill these specific work functions.

Minimum Education, Experience, Training and Licensures Required:

  • BSN required

  • Three years of job-related experience required

  • Current unrestricted licensure as a registered nurse in the state of Kentucky required.
