Location: 1106 Annapolis Rd | Suite 310 | Odenton, MD 21113 Make it Happen at Hopkins | NOW HIRING Full Time Care Manager | Hybrid Join our team and be part of a newly established care management program dedicated to serving our community by delivering high-quality, patient-centered care. The RN Care Manager is an integral member of the primary care team, working collaboratively with providers to engage patients in improving chronic disease self-management, coordinating care, and reducing unnecessary healthcare utilization. In this role, you will support high-risk patients with complex medical and social needs by coordinating care across multiple providers and settings, providing chronic disease education, performing medication reconciliation and education, and helping patients navigate the healthcare system. You will play a key role in implementing programs designed to improve health outcomes and reduce avoidable hospital and emergency department use. This is an exciting opportunity for a compassionate and motivated nurse who is passionate about improving patient outcomes, strengthening community health, and making a meaningful impact through proactive, coordinated care. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
