Position Summary

The Hematology/Oncology Pharmacist provides clinical and operational pharmacy services in an ambulatory infusion center. Responsibilities include anticoagulation management, oversight of sterile compounding of intravenous hazardous and nonhazardous medications, and provision of medication education and drug information to patients and healthcare staff. This role collaborates closely with the interdisciplinary care team to promote safe, effective, and patient-centered medication use.

Key Responsibilities

*Monitor, assess, and adjust anticoagulant therapy; counsel patients on safe and effective use.
*Oversee sterile compounding of IV hazardous and nonhazardous medications, including chemotherapy and biologics.
*Ensure compliance with USP <797>, USP <800>, and applicable regulatory standards.
*Provide medication education and drug information to patients, nurses, and providers.
*Support medication safety, quality improvement, and ambulatory infusion workflows.

Qualifications

*Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.
*Active pharmacist license in good standing (or eligibility for licensure) in the practicing state.
*Experience or demonstrated competency in oncology pharmacy, anticoagulation management, and sterile compounding preferred.
*Strong communication skills and ability to work effectively in an ambulatory care environment.

This Pharmacist will support our Outpatient Oncology and non-oncology Infusion Service in New Albany, Indiana.The Clinical Staff Pharmacist is responsible and accountable for the provisions of safe, effective, and timely medication therapy, as well as utilization of knowledge and expertise in sterile compounding of hazardous and non-hazardous medications in provision of pharmaceutical care. Additionally, this position is responsible for drug preparation and distribution of medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring chemotherapy regimens to ensure safe and effective treatment; and managing pharmacy inventory.

Qualifications

Graduate of accredited College of Pharmacy

Indiana Board of Pharmacy license

Kentucky Board of Pharmacy license

Oncology Infusion experience as Pharmacist preferred

BLS within 90 days

ACLS within 6 months
