Job Summary:

The Nurse Navigator serves as the primary point of contact for physicians and other healthcare providers, patients and their caregiver/family, and other resource-based entities, providing and accessing clinical and supportive care services offered within the multidisciplinary teams at Baptist Health and other entities as needed. The Nurse Navigator serves as an integral part of the care team within Baptist Health departments, coordinating effective and comprehensive services through the continuum of care for patients from the time of all neurology-related diagnoses, with treatment and support service options and through the recovery phase of care.

The Nurse Navigator serves as an educator, advocate, and communicator for the patient and patient caregiver/family and proactively develops processes that streamline and improve the clinical experience for both the patient and their healthcare provider(s). The Nurse Navigator will demonstrate through organizational skills the ability to provide ease and patient safety in the operational flow of the accessed Baptist Health departments.

The Nurse Navigator demonstrates the broad-based knowledge, skill, and coordination to provide nursing care and guidance to the patient in a manner that is supportive of Baptist Health's mission and vision statements. This position requires a collegial and collaborative working style to fulfill these specific work functions.

Minimum Education, Experience, Training and Licensures Required:

• Associate's degree in nursing from an accredited educational institution, or higher
• Current, non-restricted RN licensure in the state of Kentucky
• At least three years of relevant experience, neurology experience preferred
• BLS
