Description

Do you want to change lives by helping people access critical disability benefits? Join Thresholds as a SOAR Specialist with the Comprehensive Class Member Transition Program (CCMTP). In this role, you will be part of a team that works with Williams class members in their transition from specialized mental health rehabilitation facilities to living independently in the community.

Use your expert knowledge to construct each client's case for appropriate SSI or SSDI benefits. As part of building the medical summary report, you will gather supporting information and complete the applications using the SOAR method (SSI/SSDI Outreach, Access, and Recovery). You will also support your clients in getting access to other resources, including housing, community services, health care, substance use treatment, and mental health care. This includes coordinating case management and attending appointments with your client to provide support and advocacy as needed.

Ultimately, you will support the client as they transition to living independently and receiving ongoing services from a Thresholds Community Support Team. If they have not yet received SSI/SSDI at the point of transition, you will continue to work with them until they receive their benefits.

TO SUCCEED IN THIS ROLE, YOU NEED:



  • Passion for mental health advocacy
  • Excellent attention to detail and organization
  • Strong writing and communication skills
  • To be at least 21 years old
  • A valid driver's license, current car insurance, and daily access to a car
  • Willingness to provide transportation to clients
  • To acquire SOAR certification (free online training)


MANY EDUCATION AND EXPERIENCE PATHS ARE ELIGIBLE FOR THIS ROLE:



  • High school diploma or GED certificate and 5 years of supervised clinical experience
  • Active CRSS credential
  • Bachelor's degree in Psychology, Social Work, or related field
  • Bachelor's degree in an unrelated field and 2 years of supervised clinical experience
  • Master's degree in Psychology, Social Work, or related field

  • Competitive pay - Base Rate: $24.00 - $27.16 hourly / $49,920 - $56,500 annually
