Description

Position Summary

The Case Manager will provide targeted support to students navigating physical and mental health services, both on and off campus. This role involves care coordination, referral management, follow-up support, and collaboration with internal and external providers to ensure continuity of care. The Case Manager will be a key liaison between students and the appropriate campus services (academic advising, disability services, financial aid, etc.) and community resources that will support their success. This is a part-time (20-25 hours/week) position for 9 months of the year, when classes are in session.

Key Responsibilities




  • Conduct intake assessments and develop individualized care coordination plans.





  • Assist students in accessing off-campus providers for specialized care (e.g., psychiatry, eating disorders, substance use treatment).





  • Track referrals and follow-up to ensure continuity of care.





  • Collaborate with staff to coordinate referrals to appropriate campus departments (e.g., counseling, health services, financial aid, academic advising, disability services).





  • Maintain accurate documentation of sensitive and private communications.





  • Provide psychoeducation and resource navigation support to students.





  • Participate in interdisciplinary team meetings and contribute to holistic care planning.





  • Assist with crisis intervention and safety planning as needed.





  • Primary contact for CARE referrals and link to appropriate resources as deemed appropriate by Early Intervention Planning team.





  • Maintain up-to-date knowledge of local health and mental health resources.





  • Serve as a standing member of the Behavioral Concerns Team and the Early Intervention Planning group.




Qualifications


  • Bachelor's degree in Social Work, Psychology, Counseling, Public Health, or related field (Master's preferred).





  • Minimum of 2 years of experience in case management, care coordination, or student support services preferred.





  • Familiarity with HIPAA and FERPA regulations.





  • Strong interpersonal, organizational, and communication skills.





  • Experience working with college-aged populations preferred.





  • Ability to work independently and collaboratively in a team setting.
