The Supervisor, Care Management (CM) shares responsibility for the evaluation, planning, implementation, coordination and supervision of Care Management Department operations, functions and staff in alignment with corporate and departmental goals as well as applicable regulations and standards. The Supervisor is responsible for overseeing a team of clinicians and non-licensed personnel who are charged with identifying member and family needs and coordinating care in a cost effective manner.

The Supervisor is responsible for oversight of day-to-day operations, including the staff schedule and caseloads, to ensure appropriate outreach is being made. They provide expertise and support for problem-solving complex cases and issues, including utilization of real-time dashboards and reports to ensure timely outreach on referrals and member support.

The Supervisor will represent CM and attend meetings for assigned committees, workgroups and teams. They will participate in required CM department data collection, documentation audits, reporting and analysis and quality initiatives.

Qualifications:

  • Licensed Master Clinician with NYS license or Registered Nurse with NYS license required.
  • Bachelor's degree required. 4 years relevant experience may be substituted for degree
  • Certified Case Manager (CCM) credential preferred.
  • Minimum three (3) years of case management experience required.
  • Experience in a team lead or management role is preferred.
  • Experience in compiling and analyzing data required. Experience with data retrieval and statistics is preferred.
  • Familiarity with community resources and social service resources to assist patients with social needs strongly preferred.
  • Experience in home care, medical office setting or managed health plan preferred.
  • Proficiency with Microsoft Office, including Word, Excel, Outlook and PowerPoint is required.
  • Experience with CRM (Salesforce) and contact center technology (Genesys) preferred.
  • Demonstrated understanding of comprehensive care management services.
  • Demonstrated ability to lead, coach, train or supervise staff.
  • Demonstrated ability to work collaboratively with multiple partners to positively affect outcomes at the team, organization and member level.
  • Must be detail-oriented with strong organizational skills, including the ability to coordinate workflows.
  • Demonstrated knowledge with the regulatory requirements of DOH, CMS, DOI, NCQA.
  • Demonstrated ability to handle multiple projects and meet deadlines is required.
  • Excellent interpersonal, verbal, and written communication skills are required.
  • Demonstrated ability to pro-actively identify problems, as well as recommend and/or implement effective solutions.
  • Flexibility to adapt to a changing and fast-paced environment.
  • Demonstrated ability to work with and maintain confidential information.
  • Ability to travel to a variety of sites across CDPHP service area is required, including a valid driver's license and driving record showing no restrictions that would impede the ability to travel by car.

CDPHP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. In addition to cash compensation, CDPHP employees may be eligible for an incentive payment, a discretionary cash reward based on employee and company performance.
