SOME (So Others Might Eat) exists to help homeless and poor individuals and families in Washington, DC. Our more than 30 programs throughout the district provide comprehensive services that lead to mental and physical health, continued sobriety, job training, living-wage employment, and affordable housing. We meet immediate needs with food, clothing, and healthcare, and offer the tools one needs to live with hope, dignity, and greater independence.

Compensation: We offer our employees a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The hourly rate for this position is $90 - $95 per hour and may be commensurate with experience.

Position Title: Psychiatric Mental Health Nurse Practitioner (NP) - Contractor

Department: Health Services

Reports To: Medical Director / Senior Director / Senior VP of Health Services

FLSA Status: Contract

The Nurse Practitioner (NP) Contractor provides high-quality, integrated primary and behavioral health care to individuals experiencing homelessness and/or severe poverty. The NP will deliver direct clinical services, coordinate interdisciplinary care, and support compliance and quality improvement initiatives in alignment with SOME's mission and Federally Qualified Health Center (FQHC) standards.

DUTIES & RESPONSIBILITIES

1. Direct Clinical Care



  • Provide outpatient clinical services including assessment, diagnosis, treatment, and referral to specialists as needed.
  • Deliver emergency care when appropriate and document per accepted protocols.
  • Oversee disease prevention and health promotion protocols in line with established standards.
  • Collaborate with the Medical Director, Senior Director, Senior VP, clinical staff, and volunteers in daily clinical operations.
  • Review abnormal laboratory/test results and make recommendations.
  • Assess, diagnose, and treat co-occurring medical, psychiatric, and substance use disorders in collaboration with the integrated care team.
  • Provide crisis intervention, stabilization, and linkage to higher levels of care within scope of practice.


2. Care Coordination & Collaboration



  • Collaborate with primary care providers, behavioral health consultants, case managers, SUD counselors, and interdisciplinary team members.
  • Coordinate care integrating medical, psychiatric, behavioral health, and substance use treatment.
  • Facilitate referrals to higher levels of care as appropriate.
  • Participate in case conferencing and interdisciplinary huddles.


3. Documentation & Compliance



  • Complete timely and accurate EHR documentation consistent with FQHC, HRSA, and payer requirements.
  • Ensure compliance with HIPAA, 42 CFR Part 2, and other applicable regulations.
  • Support documentation for billable services under Medicaid/Medicare and other payer guidelines.
  • Document trauma-informed, recovery-oriented integrated care plans.


4. Patient Engagement



  • Provide care using trauma-informed, culturally responsive, and recovery-oriented approaches.
  • Apply harm reduction principles with patients experiencing substance use disorders.
  • Support outreach-based care delivery, including walk-in and same-day appointments.
  • Foster engagement and trust with unstably housed or unhoused patients.


5. Education & Consultation Support



  • Provide consultation and education to SOME staff and departments to support client care.
  • Deliver psychoeducation to housing, SUD, and primary care teams to promote integrated care and early intervention.


6. Professional Standards



  • Maintain active licensure, DEA registration, board certification, and malpractice insurance.
  • Participates in supervision, training, and quality improvement initiatives.
  • Adhere to professional and ethical guidelines for Nurse Practitioners.
  • Contribute to FQHC-wide quality improvement efforts to reduce health disparities.


DELIVERABLES



  • Maintain productivity of ~1.5 clients per hour.
  • Complete all patient documentation within 48 hours of encounter.
  • Participate in huddles while onsite and at least one interdisciplinary case review per month.
  • Provide quarterly reports of patient outcomes and service utilization.
  • Contribute to integrated health and SUD outcome reporting for FQHC quality measures.


CREDENTIALING REQUIREMENTS



  • Contractor must be credentialed with all DC Medicaid and MCO providers. SOME will assist with credentialing if not already in place.
  • Contractor must remain credentialed and in good standing throughout the contract.
  • If not credentialed at time of hire, the Contractor will:
  • Participate in on-site training and build clinic caseload.
  • Conduct outreach to SOME programs to build referral relationships.
  • Provide psychoeducation to staff and clients as appropriate.
  • Denial of credentialing with DC Medicaid or MCOs will result in contract termination.


Position Designation: This position is designated as Safety Sensitive. You may be subject to drug testing prior to or during your employment with SOME. In this position, you may be disqualified from employment based on the presence of marijuana in test results, even if you possess a medical card authorizing the use of medical marijuana.
