Job Summary

The Case Management Supervisor position reports the Assistant Director of Case Management, and is a dual role with 50% effort devoted to direct patient care and 50% effort devoted to leadership of the Case Management staff. This position is responsible for working with the clinical and administrative leaders of the adult services of the Case Management Department to support effective and efficient work processes for patient throughput and monitoring systems utilizing data related to hospital capacity, length of stay and clinical denials.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $133,700 - $223,500 (Annual Rate).

Department Description

Navigating through a hospitalization can be fraught with challenges as a patient transitions between care settings and adjusts to changes in their health status. At UCSF Medical Center, experienced nurse case managers in our Case Management Department are available to assist patients and their families through the confusing maze of discharge planning needs, post-acute care options, insurance questions and coverage, and medical transportation.

The Case Manager Supervisor performs lead work duties over case managers including providing on-the-job training and technical support, monitoring caseloads for accuracy in the application of program rules and regulations, and identifying areas for process improvement in order to support the case managers.

Required Qualifications

• Bachelor's degree in nursing or other healthcare profession, or a combination of comparable clinical education and / or experience.
• Registered Nurse in the State of California
• Demonstrated ability to effectively supervise a team and to manage the complex workflow and multiple priorities involved with care coordination and case management.
• Strong knowledge of case management, utilization review, discharge planning, home care and / or managed care.
• Strong knowledge of human resources management policies, with the ability to train, monitor, evaluate, and document human resources matters and performance, and to participate in decision-making on human resources matters.
• Strong analytical and critical thinking skills, with the ability to quickly analyze problems, determine appropriate level of intervention, and develop and apply effective solutions.
• Strong interpersonal skills for effective collaborations with all levels of management and staff across clinical disciplines.
• Strong written and verbal communications skills, with the ability to train and mentor subordinates, convey complex clinical and technical information in a clear and concise manner, and prepare and present a variety of reports, documentation, analyses, and project proposals.
• Knowledge of applicable business software and specialized applications and data management systems.

Preferred Qualifications

• Master's degree in a healthcare related field
• Accredited Case Manager (ACM) American Case Management Association or Certified Case Manager (CCM) Commission for Case Manager Certification

License/Certification

• Bachelor's degree in nursing or other healthcare profession, or a combination of comparable clinical education and / or experience.
• Registered Nurse in the State of California
• Current CPR certification
• Accredited Case Manager (ACM) American Case Management Association or Certified Case Manager (CCM) Commission for Case Manager Certification
