As our Clinical Manager, you will shape care delivery, guide and support caregivers, and ensure every client receives the compassion and quality they deserve.
 Your RoleYou will oversee the clinical care of an assigned caseload, ensuring optimal outcomes, compliance with regulatory standards, and strong partnerships with clients, families, and healthcare professionals. You will assess client needs, develop personalized care plans, and support caregivers in the field with expert guidance and clinical leadership.
 What You'll Do 
  
 Conduct client assessments, develop care plans, and adjust as needs evolveProvide clinical supervision and direction to caregivers and nursesCollaborate with families to set goals and align daily care with those prioritiesMaintain accurate, timely, and compliant client documentation in the EMR and home records, ensure prompt communication of status changes, and oversee information sharing with families, practitioners, and stakeholders to support continuity of care and regulatory compliance.Educate clients, families, and staff to ensure clear understanding of care needsIdentify caregiver training opportunities and provide coaching for growthSupport hiring, training, and supervision of nursing staffParticipate in on-call rotation to provide consistent 24/7 client support
 What You Bring 
  
 Graduate of an accredited nursing program, with an active, unrestricted nursing license; BSN preferredCurrent CPR certification; CPR instructor certification preferredAt least two years of nursing experience, with one or more in home health, rehabilitation, or case management strongly preferredStrong skills in clinical assessment, critical thinking, and problem-solvingExceptional communication skills, with the ability to partner effectively across clients, families, caregivers, and medical professionalsA compassionate, professional presence with the ability to thrive in fast-paced environmentsFlexibility to travel locally on short notice
