Summary: The Behavioral Health Care Manager (BHCM) is a core member of the integrated team-based care model of the primary care practices/behavioral health services. The BHCM is responsible for supporting and coordinating the behavioral health needs of patients. Responsibilities: *Work with the patient's care team to coordinate the behavioral health needs of the patients. *Screen and assess patients for common behavioral health disorders and facilitate engagement and follow-up care. *Develop a comprehensive plan of care, including self-management aspects, in collaboration with the patient &/or caregiver and the patient's care team. *Provide patient education about their behavioral health condition, medications, and available treatment options. *Guide patients in developing skills and/or strategies for managing problems and triggers to decrease relapse and reduce risk. *Provide brief behavioral interventions using evidenced-based psychosocial treatments as clinically indicated. *Participate in weekly caseload review meetings with consultant psychiatrist and communicate the resulting treatment plan to the patient's medical provider and LCSW if applicable. *Assist with coordination of appropriate internal and external referrals for behavioral health treatment/counseling for patients as clinically indicated. *Document assessments, patient progress, treatment recommendations and care plan in the electronic medical record (EMR) so as to be shared with the patient's care team. *Implement crisis intervention strategies to ensure safety and well-being of clients in emergency situations. Other information:Required *Bachelor's or Master's degree in mental health related field *2+ years of experience in behavioral health or a related field *Driver's license and access to transportation Preferred *Previous case management experience. *Proficiency in evidence-based treatment approaches such as Problem-Solving Treatment, Motivational Interviewing, Goal Setting, Cognitive Behavioral Therapy and Behavioral Activation). *Basic knowledge of psychopharmacology for common mental health disorders. *Ability to engage with patient in a therapeutic relationship. *Experience navigating an electronic medical record (such as Epic) Job Details Legal Employer: Pardee - HCHC Entity: Pardee UNC Health Care Organization Unit: PBMD Behavioral Health Work Type: Full Time Standard Hours Per Week: 40.00 Work Assignment Type: Hybrid Work Schedule: Day Job Location of Job: DR PARK Exempt From Overtime: Exempt: Yes |
