Are you experienced in a healthcare clinical setting and passionate about community health? We are seeking a full-time Population Health Specialist to coordinate and support GHC-SCW's community health programs, population health initiatives, community partnerships, and social work services. In this role, you will serve as the subject matter expert on various community health programs, including the Bridges to Access Program, Stay Healthy Visit Program, Refugee Assistance Program, and Financial Assistance Program.

Under the direction, guidance and general supervision of the Population Health Manager, the Population Health Specialist plays a crucial role in coordinating and supporting GHC-SCW's population initiatives, community partnerships, community health programs, and social work services. The Population Health Specialist actively contributes to various community health programs, including the Bridges to Access Program, Stay Healthy Visit Program, Refugee Assistance Program, Primary Access for Kids Program, and Financial Assistance Program.

In addition to program involvement, the Population Health Specialist aids in the development, monitoring and sustainment of partnerships and charitable contributions. The position actively supports the organization's health equity initiatives for both members and patients. This position is also a key contact for GHC-SCW clinic staff that assist in identifying and directing patients to other community services and resources that may be available.

JOB SPECIFIC MINIMUM QUALIFICATIONS



  1. Bachelor's degree from an accredited college or university in a relevant major, or equivalent combination of relevant education and experience is required.
  2. A minimum of two (2) to three (3) years of experience in an office or healthcare environment including customer service is required.
  3. Direct experience with community services and/or Medicaid or BadgerCare population is preferred.
  4. Previous experience using the telephone in a service capacity is preferred.
  5. Experience with medical terminology is preferred.
  6. Knowledge of and proven proficiency in the use of Microsoft Office applications including Word, Outlook, Excel, PowerPoint, Visio is required. Proficiency in SharePoint is preferred.
  7. Possession of current State of Wisconsin Driver license with a good driving record, access to a vehicle, and the ability to travel is required.
  8. Ability to work with a diverse patient population is required.
  9. Ability to occasionally work evenings, weekends, and off hours, as required.
  10. Excellent customer service skills are required.
  11. Ability to be culturally sensitive and approach members and others in a non-intimidating and supportive manner, and the ability to represent GHC-SCW in a professional and compassionate manner is required.
  12. Knowledge of or ability to learn all community resources, agencies and referral sources which are appropriate for member population groups required.
  13. Knowledge of and/or ability to learn aspects of the Medical Assistance and BadgerCare programs and the ability to learn GHC-SCW's services and benefits are required.
  14. Ability to learn and effectively use multiple computer systems including electronic medical record and scheduling systems (e.g., Epic Systems) is required. Epic experience preferred.
  15. Excellent verbal and written communication skills and the ability to document, design, and create meeting materials, general correspondence, memos, charts, tables, graphs, web page content, reports, etc., required.
  16. Outstanding customer service skills are required.
  17. Knowledge of or ability to learn about healthcare delivery systems including insurance and managed care. Ability to keep abreast of population health trends and tactics.
  18. Ability to maintain patient confidentiality required. Knowledge of or ability to learn HIPAA requirements.
  19. Strong organizational skills with the ability to prioritize, manage multiple tasks, solve problems, maintain accurate files, draft reports, and attend to details required.
  20. Ability to be flexible, meet deadlines, work under pressure, manage changing priorities, and work with minimal supervision required.
  21. Ability to work harmoniously and cooperatively with the public and all GHC-SCW staff is required.
  22. Evidence of the ability to understand and work closely with community contacts and resources is required.
  23. Ability to represent GHC-SCW in a professional and compassionate manner is required.
  24. Ability to work a flexible schedule to meet customer needs is required.
