Job Description: Position Summary The Pharmacy Clinical Coordinator acts as the principal point of contact for safe and appropriate medication use through education and physician liaison activities. Provides leadership to the department in the provision of clinical pharmacy services and drug information to medical, nursing, and ancillary staff. Facilitates the development, implementation, and maintenance of systems that promote optimal patient outcomes through safe, appropriate, and cost-effective medication therapy. The scope of this position may include inpatient and/or ambulatory pharmacy. Principal Duties and Responsibilities The following is a summary of the major functions of this individual's job
Minimum Education and Training Graduate of an accredited College of Pharmacy. Current Kentucky/Indiana Pharmacist License or eligible. Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification (if applicable) or within 6 months of start date. Completion of PGY-1 ASHP-accredited Pharmacy Residency preferred or minimum of 3 years of experience in hospital practice. BCPS preferred.
