With guidance and direction from the Community Health Worker Supervisor and the Senior Director for Care Coordination and Clinical Social Work, this role will work with patients at our UCLA Health Emergency Departments (ED). The ED CHW program engages emergency department patients who have significant unmet social needs, are experiencing homelessness and/or precarious housing, are at risk for STDs, and have substance use disorders. The CHW will provide individualized support, motivational interviewing, and navigation to medical services, housing and social services, addiction treatment, harm reduction, and other community services to address health-related social needs. The goal of the CHW is to provide navigational, case management, and peer support services to patients who present to the ED for a substance use-related visit. CHW duties include:

The University of California, Los Angeles is required to provide a reasonable estimate of the compensation range for this role. This range takes into account the wide range of factors that are considered in making compensation decisions including but not limited to experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. It is not typical for an individual to be offered a salary at or near the top of the range for a position. Salary offers are determined based on final candidate qualifications and experience. The full salary range for this position is: $30.56 - $60.82 Hourly. Qualifications
