Role and Responsibilities:


As a Rehab Counselor, you will play a vital role in providing vocational Day Program services to our program participants. Your primary focus will be on guiding individuals in their personal development, preparing them for competitive work, and exploring various employment options.



  • Provide one-on-one counseling to program participants, helping them navigate their rehabilitation journey.

  • Collaborate with Employment Specialists and Program Coordinator to establish performance goals and behavioral criteria in accordance with CARF standards.

  • Regularly assess participants' work performance and social adaptation, keeping them informed of their progress while maintaining accurate records and documentation.

  • Monitor participants' progress towards job placement readiness, communicate this to the Job Developer, and assist in preparing participants for job-seeking activities.

  • Prepare discharge summaries and conduct quarterly reviews of Individual Employment Evaluations (IEE) and other necessary documentation.

  • Actively solicit referrals and conduct intake interviews with prospective participants.

  • Adjust rehabilitation plans as needed based on ongoing evaluations of participant progress.

  • Prepare and submit reports to the Department of Rehabilitation Services, Department of Developmental Services, and other relevant social service agencies.

  • Represent the organization at conferences, seminars, and meetings related to rehabilitation program activities, as required.

  • Track departmental statistical data to assess progress towards goals and inform planning.

  • Attend to personal care needs of program participants when necessary.


Qualifications:



  • Proven experience in the disability rehabilitation field, including internships, volunteer work, or previous roles such as social worker, case manager, or mental health counselor.

  • Proven experience working in day programs, group homes, or community-based settings.

  • Associates degree in human services or related field.

  • Ability to communicate clearly and compassionately with individuals with disabilities, their families, and other professionals.

  • Ability to assess client needs and create effective rehabilitation plans and solutions.

  • Must have a Valid Driver's license.


Preferred Skills:



  • Bachelor's degree in human services or related field.

  • Holds certifications as a Certified Rehabilitation Counselor (CRC), Licensed Professional Counselor (LPC), or Certified Disability Management Specialist (CDMS).

  • Knowledge of legal protections (e.g., the Americans with Disabilities Act) and available services and resources.

  • Possess First Aid/CPR Certification.


Physical Demands:



  • The role may require standing, walking, and occasionally lifting or assisting clients with mobility challenges.

  • You may assist with tasks such as transferring individuals from wheelchairs, providing personal care, or moving equipment based on client needs.

  • Activities may involve writing, using computers, or handling various rehabilitation tools and equipment.

  • Long hours of engagement, including active participation in physical activities, may require stamina and physical endurance.

  • Tasks such as writing, typing, or assisting with mobility may involve repetitive movements.

  • Reasonable accommodations can be provided.


Environmental Demands:



  • Counselors may need to adapt to different environments, such as working indoors or outdoors, depending on the needs of the clients or program activities.

  • Noise levels can vary.

  • Lighting and temperature can vary.

  • Adhering to strict safety protocols to ensure the well-being of clients, particularly if there are mobility aids, medical devices, or potential health risks involved.
