The Transfer Center Nurse Facilitates the transfer of all emergent, urgent and elective patients into UC Davis Medical Center from all referring locations. The Transfer Center Nurse demonstrates clinical expertise, quality assessment and excellent interdisciplinary communication to ensure the coordination and timely transfer of all requested patients transfers. The Transfer Center Nurse also facilitates direct admits from UC Davis Health Clinics and phone consultations and tele-medicine requests from other facilities by connecting the referring providers with the appropriate hospital service and physician in a timely manner. #ca-ls Apply By Date January 18, 2025, by 11:59 p.m. Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position
- Valid California Registered Nurse License without restriction
- Minimum of 3 years recent experience in an acute care setting.
- Graduation from an accredited school of nursing.
- BSN Preferred for internal candidates, external candidates, and per diem conversions.
- Licensed RN with current working knowledge of discharge planning, utilization management, performance improvement and managed care reimbursement.
- Knowledge of Medicare, MediCal, and HMO legislation; Ml county contracts; third party payor contracts; Title XXII; the Joint Commission; and Lumetra criteria.
- Demonstrated knowledge of how these programs/regulations affect the functions and role of the department.
- Demonstrates the ability to incorporate them into departmentalsystems/processes.
- Knowledge of professional and ethical standards related to Clinical Case Management such as the Nursing Practice Ace, the Codes of Ethics from the American Nurses Association and the Standards of Practice & Scope of Services by the American Case Management Association.
- Skill to perform in-depth chart analysis to provide accurate information in a concise, informative manner, identify/analyze problems or issues related to areas of responsibility, developing an appropriate plan, and considering medical needs and resources available.
- Possess clinical skills and knowledge base to assess and refer to discharge planning, possible needs for care necessary to meet patient's needs on discharge.
