Overview

As a member of the interdisciplinary team, contributes professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process. Maintains a practice environment that reflects the ANA Code for Nurses, Philosophy of Nursing at UnityPoint Health and AORN standards within the structure of the Professional Nurse Practice Model and in collaboration with other care givers. The expanded functions unique to Registered Nurse First Assistant - Advanced (RNFA-A) during surgical intervention are to assume responsibility in advanced surgical assisting skills such as providing exposure, tissue handling, suturing, providing hemostasis and using surgical instruments and equipment under the direct supervision of the procedural physician who is present during the surgical procedure. Identifies the greater saphenous vein for coronary artery bypass conduit and independently performs endoscopic vein harvest. The main responsibility of the RNFA-A during Coronary artery bypass grafting surgery is to surgically remove the vein from the patient's leg using an endoscopic approach and to assist the cardiac surgeon at the chest during the operative procedure of the heart. The endoscopic approach uses small incisions and specialized minimally invasive instruments and endoscopes to internally view, cut and seal side branches and remove the healthy saphenous vein with minimal trauma to the vessel and surrounding tissue. The success of the CABG surgery is influenced by the quality of the vein and how it is handled or treated during the harvest and preparation prior to grafting. The harvested blood vessel used in CABG surgery must be free from damage to ensure proper long-term function and good patient outcomes. The surgery cannot be completed until successful vein harvest has occurred, which requires extreme level of focus, advanced skill and knowledge and exceptionally intense concentration demonstrated by the RNFA-A. The RNFA-A provides a broad range of surgical assistant skills and knowledge in the specialty of cardiothoracic, vascular and percutaneous interventional procedures which include and are not limited to all surgeries involving the heart (CABG, Valve repair and replacement, coronary artery dissection, Mass removal, LAA, etc.), open and robotic lung surgeries of all types, chest stabilization surgery, aneurysm repair and interventional cardiac and peripheral procedures. Functions autonomously in multiple procedural settings in collaboration with the procedural physician.
Qualifications

License/Certifications:
Certified Perioperative Nurse (CNOR) - Competency & Credentialing Institute (CCI); Basic Life Support (BLS) within 30 days - American Heart Association (AHA); Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)

Education:
Associate's Degree: Nursing; H.S. Diploma/GED

Responsibilities

Clinical Excellence

  • Practices within established RNFA guidelines of AORN, anticipates and plans for steps in the surgical procedure and responds to emergency situations promptly and appropriately.
  • Utilizes critical thinking in making independent judgments and maintains responsibility and accountability for the knowledge of conditions of assigned patients.
  • Demonstrates an expert and advanced knowledge base of surgical procedures and the anatomy/physiology pertinent to it as an extension to the procedural physician in the operative setting.
  • Develops an individual plan of care using a collaborative approach with the health care team.
  • Assumes a leadership role for the care team in conjunction with the procedural physician in the execution of the procedure and throughout the procedure.
  • Delegates and provides direction for other healthcare providers and holds them accountable for their involvement in the plan of care.
  • Skillfully applies principles of asepsis and infection control. Maintains the integrity of sterile field. Drapes operative areas. Ensures these principles are applied to the entire operative setting and operative team.
  • Preparation and administration of medications preoperatively, as directed by the surgeon and anesthesiologist.
  • Assume responsibility in advanced surgical assisting skills such as providing exposure, tissue handling, suturing, providing hemostasis and using surgical instruments and equipment under the direct supervision of the procedural physician.
  • Able to skillfully and expertly handle and manipulate endoscopic equipment.
  • Provides exposure through appropriate use of instruments, retractors, suctioning, and sponging techniques.
  • Handles and dissects tissues in collaboration with the surgeon.
  • Clamping blood vessels, coagulation bleeding points, ligating vessels and other procedures in collaboration with the surgeon to achieve hemostasis.
  • Independently sutures muscle, fascia, subcutaneous, and skin tissue in collaboration with surgeon.
  • Places drains as surgically indicated.
  • Assists with cannulation and de-cannulation procedures.
  • Assists in delivery of primary health care services and patient care management.
  • EVH skills: identify vein, dissection and removal of vein, preparation of vein


Quality of Care

  • Promptly completes documentation to assure an accurate legal record of patient's care.
  • Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents, infections and reporting potential or actual concerns immediately.
  • Delivers high quality care utilizing the nursing process while demonstrating knowledge and clinical skills of the specialty required on assigned unit in accordance with established policies, procedures and standards of care.
  • Provides education to the care team to improve operative processes to achieve better patient outcomes, ensure patient and team safety and to improve effectiveness and efficiency.
  • Takes an active role in STS, NCDR, GWTG data review and participates in action planning for improvement of scores and outcomes.


Department Support

  • Continues personal and team professional development. Is involved in professional activities that enhance nursing practice by sharing knowledge through networking and research and participates in developing this in the team.
  • Delivery of continuing education, assessing educational needs and presenting in-service activities as assigned.
  • Responsible for planning and initiating projects related to process improvement, professional growth, leadership and quality improvement within department on an ongoing basis.
  • Collaborates with other team members to develop, coordinate and implement a perioperative plan of care unique to individual patient needs.
  • Ability to work as a team member.
  • Collaborates with perfusion, anesthesia, CVICU, Cardiac Step-down unit, cath lab, main OR on patient care and processes.
  • Participate in the on-call schedule- RNFA call for vein harvesting, and RN call to meet team needs, Twenty hour call is shared between a limited number of team members therefore call expectations will be up to or exceeding 185 days per year.

Compensation and Benefits

The compensation range for this position is $38.61per hour - $66.41per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
