About the unit Little Heroes League is the only care coordination program embedded at top-ranked Level IV NICUs across the United States. Nurses and social workers are dedicated exclusively to these babies and serve as the point person to organize care for each of these children and to help the needs of their families. Acting as an advocate for each baby, they coordinate multiple pediatric specialists, coordinate procedures and anticipate what each child needs. They are also adept at navigating the health care system, which can be a daunting task for parents of children with complex health needs. They are the central contact and source of comfort for these families, whom they will also support by identifying social service and health care agencies closer to home and conducting home visits. https://littleheroesleague.org/helendevoschildrenshospital About Outpatient Clinics Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. Scope of work Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes. Qualifications Bachelor's Degree Nursing Required Master's Degree Nursing Preferred 5 years of relevant experience previous nursing or CDE clinical experience Required Prior teaching with patients of all ages Preferred LIC-Registered Nurse (RN) - State of Michigan Required CRT-Basic Life Support (BLS) - AHA American Heart Association or ARC American required within first 90 days How Corewell Health cares for you Comprehensive benefits package to meet your financial, health, and work/life balance goals.
