Bilingual differential for qualified candidates.

This is a part-time, 19.5 hours per week position. 2 7.5 hour days and 1 4.5 hour day per week. Days worked per week are flexible.

SUMMARY

The Case Manager position provides family-centered, client-focused, relationship-based case management services to pregnant teens, teen parents, their children and other family members under the auspices of the statewide Standards of the Adolescent Family Life Program-Positive Youth Development (AFLP-PYD), with additional state regulations for the Cal Learn program. This position effectively utilizes Agency policies and procedures and makes appropriate independent judgments in the course of daily work.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Supports and promotes the mission of the Agency: Foothill Family empowers children and families to achieve success in relationships, school, and work through community-based services that advance growth and development.
  • Provides case management services according to the Standards of AFLP PYD and Cal Learn regulations, as reflected in program manuals. Activities include but are not limited to networking, outreach, enrollment and intake, assessment and reassessment, service planning, case management, including referral, transportation to services and advocacy, monitoring and evaluating client progress, updating service plans, data collection and record keeping, special group events for clients.
  • Drives or arranges transportation for clients, parents and children as appropriate and necessary to ensure that service needs are met.
  • Drives to clients' home, school, community or Agency offices to provide field-based services to clients and families as assigned and based on the families' needs and availability or to meet with school personnel.
  • Carries a full caseload of twenty five (25) for AFLP PYD and forty (40) for Cal Learn.
  • Identifies gaps in and barriers to service, documenting, and communicating these to the program manager and participating in the development and implementation of responses.
  • Keeps abreast of new developments in the field of child and adolescent issues, teen pregnancy and parenthood, teen pregnancy prevention, and other related areas.
  • Conducts self in a professional manner, according to the policies and guidelines set forth by Foothill Family and the Social Work Code of Ethics.
  • Collaborates effectively with staff from other programs on joint clients.
  • Attends in-person meetings and events at various locations within the Los Angeles County and surrounding areas.
  • Displays sensitivity to the service population's cultural and socioeconomics characteristics.

POSITION REQUIREMENTS

  • BSW; BA in a social science or 2 years Case Management and/or mentoring experience with youth.
  • Excellent verbal and written communication skills; ability to conceptualize.
  • Ability to document work in the client record, to write other documents related to client work and to complete administrative reports with accuracy and timeliness.
  • Possess organizational skills to manage high activity level, provide needed services and attention to each client while maintaining all reporting and documentation requirements in a timely and accurate manner.
  • Excellent people and problem solving skills; ability to work independently as well as a member of a team.
  • Ability to work with staff and clients from diverse ethnic, cultural, and economic backgrounds, and to provide services in a non-judgmental manner.
  • Affinity for working with adolescents and young children.
  • Initiative, flexibility, creativity, and enthusiasm.
  • Valid CA Driver's License and maintains insurability on the Agency's auto liability policy (including a minimum of 2 years driving experience after receiving license) and maintains the California state required auto insurance liability limits.
  • Bilingual English/Spanish skills required.
