Job Summary: Position functions as the Continuity of Care Coordinator with the local State Mental Health Facility (SMHF) through the Utilization Management department of the Network Development and Management division of Integral Care, the Local Mental Health Authority (LMHA) for Travis County. The purpose of the position is to perform highly complex (senior-level) coordination work. Work involves coordination and linkage activities to community-based services and assisting in developing client goals and objectives. Works under general supervision with considerable latitude for the use of initiative and independent judgment. Performs moderately complex local authority/coordination work assisting clients in accessing/linkage to medical, social, educational, financial, and other appropriate services/supports that are consistent with the individual's recovery goals. Provides coordination and discharge planning to assure that the individual is discharged to the least restrictive environment

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:


  • Graduated from an accredited college or university with a Bachelor's degree, with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.

License:

Required:


  • Current and valid Driver's License and a clean driving record.

Experience:

Required:


  • Human, behavioral or social services experience.

Hours: Full-Time.

Workhours: 8:00AM - 5:00PM.

Workdays: Monday - Friday.

Salary: $23.25 - $24.41 an hour based on education.
